Fixed Term contract for 18 months

One Bromley are Health & Social care service working together with the community and voluntary sector to provide a more joined up approach & care for people who live in Bromley.

One Bromley brings together a number of local health providers such as Kings College Hospital, Oxleas NHS Trust, Bromley Healthcare, Bromley GP Alliance, St Christopher’s, Bromley Council, SEL CCG & Bromley third sector enterprise to work more formally and deliver seamless, personalised & joined up care for individuals.

Working together in a more joined up way enables us to absolutely focus on meeting the needs of patients, rather than patients having to move from one part of the health & care system to another to get everything they need.

One Bromley are currently recruiting a Digital First Change Manager, this role is a key part of the Digital First programme led by South East London CCG.

This role will work in Bromley borough and specifically with Bromley GP Practices and PCNs. The postholder will be the lead for Bromley into South East London CCG Digital workstreams.

The postholder will have an important role in providing support to primary care providers in the implementation of the Digital First Programme, delivering transformational change leading to better outcomes for patients.

 

The postholder will work primarily within their borough (Bromley) as an integrated part of the primary care teams. However, they will also work together across the Collaborative sharing expertise and knowledge cross-borough as is required.

 

The postholder will also contribute to planning for continuous reduction in health and wellbeing inequalities across all boroughs of the CCG in collaboration with the wider system.

Please note that we are particularly interested in candidates who can demonstrate:

 

  • Knowledge of health service management, including change management & workforce redesign
  • Experience of working in a healthcare setting & specifically in Primary care.
  • Experience of working successful mobilisation projects within an NHS organisation.
  • Understanding of digital within primary care
