Job summary  

We have an opportunity for you to join our innovative Health Support for School Service (School Nursing) team in Bromley.

Bromley Healthcare is seeking to recruit a highly motivated and innovative practitioner to join the Health Support for schools team. There are opportunities for full or part time posts.

The Health support for school service consist of skill mix that are structured to deliver core and targeted service to children and Young people who attend a maintained school, PRU, college, youth offending service or home educated .

The post holder will be the named School Nurse Health Advisor for an integrated care network and the schools within that area. This role will incorporate a supervisory role for staff nurses who work within the Health Support to Schools team.

The post holder will work as a member of the Health Support to Schools Service and will work with schools, families and the wider community to improve the health of school aged children identifying the more vulnerable, and developing clear referral pathways so that all CYP to reach their full potential.

Main duties of the job  

Keeps timely, accurate, comprehensive and contemporaneous records, ensuring confidentiality at all times

Adheres to the NMC Codes of Professional Conduct and to be conversant with the Scope of Professional Conduct and other NMC advisory papers.

Participates actively in continuing education programmes and accepts responsibility for professional and self-development including requirements for PREPP.

Maintains own professional folder in line with revalidation procedure, keeping this up to date and recording all necessary information

Accesses and keeps up to date with all BHC mandatory training requirements. Participates in BHC's appraisal process

Completes appraisals and development of personal plans for skill mix and junior members of staff

Accesses regular clinical supervision

Clinical

Provides a high quality and skilled Health Support to Schools service to children and young people living in Bromley.

Is responsible for the management of skill mix staff ensuring service delivery is efficient and timely.

Acts as a resource and liaison between parents, children/young people, education personnel, health, social care, voluntary and other agencies breaking down barriers and improving the sharing of information, ideas and expertise.

Supports school staff to profile the health needs of the school age children, their families, schools and their community using this information to inform commissioners and providers

Promotes the concept of health and its benefits, encouraging self-help and healthy lifestyles in children, young people and their families.

Develops training sessions for schools to meet the training needs as identified by the schools themselves. Ensuring that school staff are competent and confident in their ability to support CYP with medical conditions. School training plans put in place and information of same kept to share with commissioners.

Supports school staff to identify and implement the health care plans for individual children, providing information and advice for further support/training so that staff can recognise and act quickly when a problem occurs.

Meets with identified person in schools at least termly to address health needs of the school.

Working with the service lead they will develop appropriate data bases, contribute to reports as necessary reporting any issues/concerns that may arise and making sure that the service meets all targets in the service specification

Works in partnership with the local Health Visiting/FNP service to ensure seamless care as the child progresses from the early years into school.

Adheres to and works within child safeguarding policies National, BCSB & Bromley Healthcare in the prevention and early detection of abuse in all age groups.

Develops accurate factual reports, attends case conferences, other multi professional meetings as required for identified children and their families.

Writes comprehensive court reports if requested.

Meets with all identified children at least once a term, refers/signposts to other professionals/ services if identified.

Works in partnership with primary care, ensuring that where appropriate, the relevant GP practice is notified of CYP health concerns, to secure an integrated system of care that safeguards them.

Works in partnership with other key stake holders to promote the welfare and safety of CYP.

E.g. contributing to keeping CYP safe from the dangers of radicalisation and extremism and promoting safe practices and a culture of safety, including e safety.

Offers strategic support to schools in order to contribute to reducing the number of children who enter the safeguarding system through preventative work.

Targets work with identified vulnerable groups e.g. Children Looked After, Young Carers to meet all their health outcomes offering these CYP an annual meeting.

Signposts school staff to appropriate training materials to help them deliver PSHE

Works in liaison with identified school staff to contribute to the delivery of health improvement, promotion and preventative approaches through signposting to other healthcare professionals/ services and available resources.

Promotes emotional well-being through the school aged years working alongside children and young people to support those with emotional and mental health difficulties and referring to CAMHS where appropriate.

Has knowledge of other available resources to support emotional and mental wellbeing e.g. on line, training packages.

Support schools to adopt a comprehensive whole school approach to social and emotional well being.

Work in partnership with primary and secondary care colleagues to support children and young people with long term conditions or complex needs, signposting and referring school staff to appropriate support and to and facilitate correct management of health conditions to ensure hospital admissions are kept to a minimum.

Has a clear defined link into the schools senior management team via a named member of staff identified by the school.

Works with the senior management team to develop a comprehensive Action Plan for the school which includes all policies, procedures and guidelines, overview of SEN, safeguarding and pastoral and medical needs together..

Ensures services are delivered in a culturally sensitive manner adapting the service provision accordingly to promote equal access to services eg use of interpreters, flexibility of appointments offered.

Works to and maintains school nursing standards as agreed by Bromley Healthcare, informing the Service Lead when agreed standards cannot be met.

Develops and maintains professional links with similar services in other areas in order to share information and inform best practice.

Implements regular team meetings to promote effective communication and team working Leads by example promoting best practice and high standards at all times.

Works with the Service Lead to review service delivery to ensure the service meets both national and local requirements.

Works with the Service Lead to ensure that all policies, procedures and guidelines are monitored, regularly updated and are relevant to the service

Ensures that all incidents are reported appropriately using Datix and that the Service Lead is informed of these at all times.

Manages skill mix staff effectively, addressing issues such as poor performance and informing the service lead of these issues in a timely fashion.

Carries out risk assessment for children with health needs in school

Education

Participates in the orientation and induction of new staff, promoting good interpersonal

relationships within the team.

Acts as a mentor in the education and training of pre and post -registration students. Facilitates other students as required. Acts as a preceptor for newly qualified nurses/ specialist practitioners into the Health Support to Schools service

Supports new staff to develop appropriate skills and knowledge to understand how the service meets and contributes to performance targets.

Ensures the quality of the local learning environment and student experience Implements programmes of induction for new staff, students both pre and post reg. Undertakes such training as may be required to meet the needs of the service.

Acts as a resource for skill mix staff offering guidance, support and supervision

Identifies own learning needs and that of skill mix staff within the team and actively finds solutions to address this

Remains up to date with current professional issues e.g. by attending professional forums, reading professional journals etc.

To work with the Service lead in reviewing the service being offered looking at smarter ways of delivery.
